Participant waiver & parental consent

1. ACKNOWLEDGMENT AND ASSUMPTION OF RISK

I understand that participation in Gridiron Girls Flag Football clinics, camps, practices, games, tournaments, training sessions and related activities involves inherent risks, including falls, collisions, physical contact, sprains, fractures, concussions and other injuries. I understand that not all risks can be eliminated and voluntarily consent to my child participating with awareness of these risks.

2. RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by applicable law, I agree to release Gridiron Girls Flag Football and its owners, directors, officers, coaches, employees, volunteers, representatives, sponsors and affiliated organizations from claims arising from ordinary risks of participation or ordinary negligence, except where prohibited by law. This provision does not waive claims that cannot legally be waived, including gross negligence, reckless conduct or intentional misconduct.

3. MEDICAL AUTHORIZATION

I certify that my child can participate, subject to any restrictions or accommodations noted below. In an emergency, I authorize program representatives to contact emergency medical services and seek appropriate assistance if I cannot be reached promptly. I understand I am responsible for medical expenses to the extent permitted by law.

4. PARTICIPANT CONDUCT AND SAFETY

Participants must follow program rules and coaches' instructions and demonstrate sportsmanship, teamwork and respect. Gridiron Girls Flag Football may remove a participant when necessary for safety.

5. OPTIONAL PHOTO AND VIDEO CONSENT

I understand that Gridiron Girls Flag Football may use photographs or videos for its website, social media and promotional or educational materials. Declining consent does not affect eligibility to participate.

6. PARENTAL CONSENT AND ACKNOWLEDGMENT

I confirm that I am the parent or legal guardian of the player named in this form; that I have read and understand the information above; and that I voluntarily consent to my child's participation. I understand this document is subject to applicable law.

Registration and medical information are used by authorized Gridiron Girls staff to administer the clinic and support participant safety. You provide the payer’s email and payment information directly to Stripe during checkout. Card numbers and security codes are not stored by this website.
